Ok so I saw this on another forum and wanted to see what stories you guys would have.

What was your biggest fail story and what was the aftermath?

and by fail I mean something bad like forgetting to tighten your lugnuts and having a wheel come off or forgetting to tighten your brake and remembering when its too late
