the grease you should be using lithium grease. cost should be around from $3-8 bucks a can. you can find them anywhere at a auto store or any hardware store.

Call your local nissan dealership and ask them for the part #.

you can try going to courtesyparts.com site and use their online visual part locater (whatever you call it). I think the last time i checked it was part #39640 for the repair kit. I think you can get the boot separately
