s13 Brake Problem

So today, with help from a friend, I changed out my rear brakes. Everything went smoothly (minus the broken ratchet, but that is another story) and when everything was put together, I tested out the brakes and started the bed them in per the manufacturers specs. I drove about a city block when I heard this horrible clunking and grinding sound coming from the rear. It turns out that the rotor where the pads make contact are completely split from where the rotor bolts to the hub.
Now could this just be from bad OEM replacements or could there be something catastrophically wrong with my caliper? Any input is appreciated. Thank you.
