I know that the '91 was the first year of the DOHC, and the front bumper changed between the two years, but what else is different between the cars? What is different to keep a 1990 from getting the Injen CAE?

DOHC and SOHC engines are set up differently. Different locations for the intake manifold, etc. Different front bumper putting the CAE in a different spot (it sits in the fender in front of the left tire).

Many other differences

91 has 15 more hp and a like number more torque.
91 has options like HICAS and LSD
91 has adjustable headrests on the front seats
91 has a little '1' after the '9' in the year
