Need to make a survey for class. Which program to use?

I need to make a small survey, about 10-15 questions that have answers that ppl will choose, and maybe some small text input (like if they press other, and can enter a short explanation). I am guessing hopefully up to 100 people will answer it. Which program should I use? I want it to be simple and keep track of results in like a spreadsheet/database form so I can analyze them.
