Sanus, not ANUS wall mount kit for Flat Panel TV's 30"-50". According to the box, it holds up to 130lbs. Tilt's +/-12 Degrees.

All parts should be in there, just opened the box to take the pictures.

Part Number is PLA50b-01. Retails at Target for $120...
