Camera: Sony Cybershot
Mounting device/method:I used aluminum square tubing. It was setup on the passenger headrest posts. There was one bar in front and one behind the posts, and they were clamped together with U bolts. I just drilled a hole in one of the tubes, where I could insert a bolt to hold down the camera.
Cost:: about 10$
Review: My first impression was that for 10$, this setup looked nice and sturdy. The only movement that came was from the seat. What I found out was that for some reason, with my setup, there was a lot of vibration transmitted to the camera which made for some really annoying videos. I tried to remedy that by putting foam or little cushions between every metal part of the stand, and I even tried putting some under the camera where it contacted the mount. That didn't help very much, but for the price it did what it was supposed to.
Unfortunately, I do not have any videos or pictures of the setup at the moment.

Sony TRV-950 with steadyshot off.
I/O Port rollbar mount (http://www.ioportracing.com/Merchant2/merchant.mvc?Screen=PROD&Product_Code=CM&Category_Code=IOPP)
Autopower rollbar.Depending on how you set it up it can be shaky or not, if you use the steadyshot its absolute crap. If you make the camera mount compact it vibrates less then if you really have to extended to get the shot.
I had it a little more extended then I usually would because I was in a rush setting it up.

Not an in-car vid, but I did use my basic camera mount to mount my camera to my roof!! (I got a friend to hold it through the passenger window just in case!)
The camera is a Sony Handycam DCR-HC19E PAL
The camera mount is the SM2 on this page:

Video (not sure how to embed):

Edit - it embeds it automatically; very cool!
I forgot to answer your questions.
