Engine - Blacktop sr20det
Boost controller - Greddy Profec b Spec II
Settings.
Hi - Gain 5%, Set gain 1.25, Warning 1.05, Warning L. 14%
Lo - Gain 5%, Set gain 0.22, Warning 0.60, Warning L. 0%
Car battery went dead over winter. Lost settings.
Stock t25, injectors, 3 inch exhaust.
Problems - at 4-5 thousand RPM the boost controller beeps and limits the car.
I have tried reading the manual and online guides and my buddy has tried helping me and we can't figure it out.

If it beeps/flashes red that means you have exceeded the limit that you set on your boost controller. If you set the limit to 10 psi and you hit anything above that, it will drop the boost to whatever percentage you set it to drop. It's a safety feature.
Try lowering your set value as well as your gain value. I own the same one and had a hard time with it. I still can't quite get it that way I want it.
EDIT: Wait, your set gain should be way lower than your warning and warning limit. You have the warning below your set gain. Your set gain should be somewhere around +- 4 psi from your desired boost from what I've read. What boost level are you trying to accomplish?

On low, you can leave your set value at 0 for around 7 psi which should be stock boost.
I have the same setup and I run just below 14 psi. I have my Set value up pretty high, like 70 or something. My gain is somewhere between 15 and 20, and I can't remember where my set gain is, probably somewhere above 10 psi. You're warning should be set somewhere above 14 psi on a stock T25. Mine is at like 14.5 or 15. The limit should be something reasonable, like 10%-15%, something that will drop it into a safe range should you over-boost.

You have your boost controller turned onto PSI instead of KPA?

Yes, if you want PSI you have to manually change it. It says somewhere in the manual. Even when you change it to PSI though, it reads PSI*10. So like... 10 PSI reads as 100 PSI, 15 PSI reads as 150 PSI, and so on. Kind of annoying but you get used to it.

u sure the vacuum line going to the wastegate actuator didn't pop off?

Yeah, or maybe you have it hooked up to the wrong fittings on the boost controller solenoid. Make sure it's hooked up to the one that's for internal wastegates. If it's connected to the external wastegate fitting, it will never stop building boost, the wastegate will stay shut.

Checked it, everything is hooked up

I don't know then. Make sure your wastegate isn't bad? What are your boost settings now since you changed them?
